Fraser Health CARES team

Fraser Health CARES (Community Action and Resources Empowering Seniors) is an evidence-based initiative that promotes healthy aging and decreases the impact of frailty on health care resources. CARES collaborates with primary care and acute care, assisted living, home health, researchers, community organization and EMR vendors to integrate holistic supports for healthy aging throughout the health care system. We are currently integrating social prescribing into the Fraser Health community via collaborative partnership.
